This workshop is for Edmonton Southside Primary Care (ESPCN) patients with limited space available for non-ESPCN patients. The workshop is for people over the age of 18 who wish to complete their Personal Directives and Enduring Power of Attorney legal paperwork. Please note, participants must be a resident of Alberta and have government-issued photo ID to participate.
Join a specially trained Healthy Aging Social Worker and Commissioner of Oaths who can help answer your questions and provide insight from a medical perspective.
During this workshop you will:
- Learn the importance of having these documents
- Receive step-by-step instructions on how to fill out the paperwork
- Work with the Social Worker to complete the forms properly
- Have the finished papers witnessed and signed, making them official
If you have any cognitive, hearing or mobility concerns, or any other challenges being in a group setting, please ask your physician for a referral for a one-on-one appointment.

Due to the legal nature of the Enduring Power of Attorney documents, we can only provide this service in English and registrants cannot rely on an interpreter. Please ask about options for Personal Directive workshops in other languages.
