Advance Care Planning is a workshop that provides information, education, and guidance on the processes for completing Goals of Care (GOC), Personal Directives (PD), and Enduring Power of Attorney (EPA) documents.
Join an ESPCN social worker who can help answer your questions and provide insight from a medical perspective.
During this workshop you will:
- Learn the importance of having these documents.
- Receive instructions on how to fill out the paperwork.
- Receive information about available resources in the community.
