This workshop is open to anyone over the age of 18 who wishes to complete their Personal Directives and Enduring Power of Attorney legal paperwork. Please note, participants must be an Albertan resident and have picture ID to participate.
Join a specially trained Healthy Aging Social Worker and Commissioner of Oaths who can help answer your questions and provide insight from a medical perspective.
During this workshop you will:
- Learn the importance of having these documents
- Receive step by step instructions on how to fill out the paperwork
- Work with the Social Worker to complete the forms properly
- Have the finished papers witnessed and signed making them official
Family members or a support person are welcome to join the workshop but they must register as space is limited. If you have any cognitive, hearing or mobility concerns, or any other challenges being in a group setting, please ask about one on one appointments.
Format: Currently being offered by individual appointment only.
Time commitment: One time. Two hours long.
Important note: Due to the legal nature of the Enduring Power of Attorney documents, we can only provide that service in English and registrants cannot rely on an interpreter. Please ask about options for Personal Directive workshops in other languages.
