Geriatric Care 

Care for Older Adults
Some seniors require not only medical support, but also social, caregiver or financial assistance. The PCN aims to provide comprehensive care to seniors through the services of a multidisciplinary team working with family physicians. Our goals are to help older adults remain healthy, active and safe within the community for as long as possible and to prevent crisis situations from developing.

The PCN’s Geriatric team includes several professionals trained in geriatric care.  A Clinical Nurse Specialist, nurses and a social worker provide support to seniors in collaboration with the family physician and clinic nurse. Care for older adults may also involve the service of an occupational therapist through our partnership with Alberta Health Services.

The Geriatric team acts as an extension of the clinic to care for the patient in the community. Using their contacts and linkages within the community and with Alberta Health Services, the team ensures all available resources are coordinated for the patients and their families.

Access to the PCN’s services is only available the patients of family physicians who are members of the Network.
