The patient - physician relationship is the most important relationship in family practice. The PCN’s multidisciplinary team of professionals enhances that relationship and supports comprehensive care.
The Edmonton Southside PCN has successfully established small multidisciplinary teams at most clinics. Teams consist of primary care nurses or nurse practitioners, social workers and dietitians.
The nurses provide in-clinic support with a focus on chronic diseases and geriatric care. They also provide basic nursing services to the practice’s patients as needed. The social workers and dietitians provide assistance at the clinics as needed and when time and space permit or they work in the community at locations convenient for the provider, the patient and for the service being provided.
The team members provide and support patients care in collaboration with patients’ family physicians. They contribute to the delivery of comprehensive care following best practices with the patients’ goals in mind.
Access to the PCN’s services is only available the patients of family physicians who are members of the Network.
