Chronic Disease Management 

Chronic Disease Management is a collection of lifestyle, self-management and health care services designed to support a persons overall health. Our clinical staff members (nurses, dietitians and social workers) have expert knowledge of many chronic diseases and can help patients better manage their lives with diseases like diabetes, dyslipidemia, smoking, obesity and hypertension.

Chronic diseases are conditions that are rarely, if ever, cured and if not managed correctly, may worsen. These diseases may have an impact on the physical, emotional or mental health of patients and daily routines to manage the disease may be difficult to follow.

All of us should take care of our selves by staying active, eating healthy and following medical instructions. But, for people with chronic diseases, it becomes extremely important to take actions in maintaining health at home, between visits with their health professional. The patient needs to fully understand their disease and have the resources and supports to manage it effectively.
