About Us 

Edmonton Southside Primary Care Network staffThe Edmonton Southside Primary Care Network (PCN) is a group of close to 138 family physicians and 53 PCN staff (nurses, social workers, dietitians, exercise specialist and administrative staff). These professionals work with Alberta Health Services and other health care professionals to improve, coordinate and deliver primary health services for their patients in the family practice environment.

The PCN was established to be a resource to physicians through the creation of multidisciplinary teams with the goal of providing more comprehensive family medicine. Long-term, the PCN intends to show that this strategy will improve health outcomes in the practice population and reduce overall health system costs.

Primary care nurses and nurse practitioners are assigned to member clinics and work closely with the physicians to provide patient care.  They are trained in diabetes, cardiovascular risk, obesity management, tobacco reduction and geriatric care, and will be instructed on additional chronic diseases over time. 

The in-clinic nursing services are supported by two referral programs; geriatric care and mental health.  The geriatric team provides linkages to geriatric services within the region, conducts home visits and considers caretaker support issues.  Patients with psycho-social issues can be referred to a team of social workers.  This team of mental health coordinators conducts a thorough assessment of each patient and helps them find the support services they need in the community.

Dietitians provide half-day clinics across the PCN and engage patients through individual and group appointments.  They provide nutrition counselling including education and lifestyle and behaviour modification strategies for the management of chronic diseases such as diabetes, and are available for any dietary or nutritional concern.
