The Edmonton Southside Primary Care Network (PCN) is a group of close to 138 family physicians and 53 PCN staff (nurses, social workers, dietitians, exercise specialist and administrative staff). These professionals work with Alberta Health Services and other health care professionals to improve, coordinate and deliver primary health services for their patients in the family practice environment.

The PCN was established to be a resource to physicians through the creation of multidisciplinary teams with the goal of providing more comprehensive family medicine. Long-term, the PCN intends to show that this strategy will improve health outcomes in the practice population and reduce overall health system costs.
Primary care nurses and nurse practitioners are assigned to member clinics and work closely with the physicians to provide patient care. They are trained in diabetes, cardiovascular risk, obesity management, tobacco reduction and geriatric care, and will be instructed on additional chronic diseases over time.
